The Preventive Care Benefits Plan delivers real §213(d) qualified medical care to employees through a structured wellness program with favorable tax treatment for both employers and employees.
Each enrolled employee, along with up to 7 household members, receives continuous access to virtual primary care, virtual urgent care, mental therapy counseling, prescription coverage on 4,600+ generic medications, lab testing, and ongoing physician-directed care management at no out-of-pocket cost.
The program operates through two parallel transactions. The employee's pre-tax salary reduction funds participation in the wellness program. The plan delivers medical care through licensed clinicians and incurs the cost of that delivery. The §105(b) reimbursement corresponds to the actuarial value of the medical care delivered, independently established at 54.18% net actuarial value.
The plan pairs with an employer's existing major medical coverage and is structured to align with IRC §125, §105(b), §213(d), and the participatory wellness framework at 42 U.S.C. §300gg-4(j)(1)(B).

Employee Eligibility
Full-time employees averaging 30+ hours per week and earning a minimum of $26,000 per year qualify for the program. Employees currently enrolled in Medicaid are not eligible.
Employer
Employers realize an average of $1,186 in net FICA tax savings per W-2 employee per year. This is a tax saving — not a credit — recognized at every payroll run. There are no admin fees on employer savings.

Employees average $250–$350 in monthly tax savings, which are then repurposed to fund expanded preventive benefits. Net take-home pay is designed to remain neutral.
